29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about 02/19/2013, on the west side of the Mel Ott Park Recreation Center construction site, the employer allowed the employees to operate an aerial lift without a fall arrest system in place to prevent being catapulted from the lift wihile driving.
