29 CFR 1910.106(d)(4)(v): In every inside storage room there shall be maintained one clear aisle at least 3 feet wide. Containers over 30 gallons capacity shall not be stacked one upon the other. Dispensing shall be by approved pump or self-closing faucet only:  (a) Flammable Storage Room. - The 55 gallon drum of unsaturated polyester base resin styrene, a flammable liquid, did not contain an approved pump for dispensing the liquid, on or about February 28, 2013.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire:   (a) Flammable Storage Room - Employees dispense contents from the 55 gallon drum of unsaturatured polyester resin styrene, a class 1C flammable liquid, into a metal bucket to be used in the mixing of the cultured marble product. The nozzle and the drum were not electrically interconnected, on or about February 28, 2013.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.107(b)(5)(i): The spraying operations except electrostatic spraying operations shall be so designed, installed and maintained that the average air velocity over the open face of the booth (or booth cross section during spraying operations) shall be not less than 100 linear feet per minute. Visible gauges or audible alarm or pressure activated devices shall be installed to indicate or insure that the required air velocity is maintained:  (a) Ecumar, Inc. - The open face of the spray booth did not measure 100 linear feet per minute. The average velocity over the face of the booth was 68 linear feet per minute, on or about February 28, 2013.   Abatement certification required within 10 days after abatement date. 29 CFR 1910.138(a): The employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes:  (a) Ecumar, Inc. - The employer did not select the appropriate hand protection for employees who are exposed to solvents such as but not limited to frekote and cadox M-50A which can cause severe skin irritation. Employees wore Exact powdered latex gloves, style 7722P, that ripped during use which allowed the chemical to contact the employee's skin, on or about February 28, 2013.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) Ecumar, Inc. - The mixing/work area contained various solvents, marble dust/powder, pigments, catalyst and other work material. The employer did not have a housekeeping program in place, on or about February 28, 2013.   (b) Ecumar, Inc. -  The area in the sanding room contained marble dust from sanding the molds. The fine dust particles coated the table, collected on the floor and underneath the 55 gallon drum in the corner from the sanding operation, on or about February 28, 2013.
