29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    a)  On or about February 11, 2013, employees were exposed to fall hazards of greater than 8 feet because the scaffold they used was not fully planked.  Only one plank was installed at each working level of the scaffold.

29 CFR 1910.134(c)(1):  In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not update the respiratory protection program as necessary to reflect those changes in workplace conditions that affected respirator use:    a)  On or about February 11, 2013, and at times prior, employees were potentially overexposed to air contaminants such as asbestos because the employer had not updated the written respiratory protection program to reflect the current OSHA permissible exposure limit (PEL) for asbestos of .1 fibers/cc.  The program stated that the PEL was .2 fibers/cc.
