On or about February 28, 2013, on a 5:12 pitch roof, employees using a pneumatic nail gun did not wear eye protection, exposing himself to being struck by debris.

Wells Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313876187, citation number 01, item number 002 and was affirmed as a final order on May 13, 2010, with respect to a workplace located at 3627 St. Nicholas Avenue, Jacksonville, Florida 32207.    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. On or about February 28, 2013, on the 5:12 pitch roof, employees installing roofing shingles were not protected from an 8-foot 6-inch fall hazard by the use of conventional or alternative fall protection systems.
