29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings: On or about February 28, 2013, two employees were exposed to fall hazards of 12 feet to the drop pit below in that the guard rail system was not completed. The middle section was guarded by just caution tape. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
