29 CFR 1904.4(a): Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that:    The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:      2010 OSHA Form 300 - On or about 7/17/10, an employee was severely burned, which resulted in days away from work that were not recorded on the Log of Work-Related Injuries and Illnesses.    Auction & Recovery Services, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 1904.01(a), which was contained in OSHA Inspection number 314539990, Citation 2 Item 1, and was affirmed as a final order on 7/13/2010, with respect to a workplace located at Hermon, Maine.

29 CFR 1904.29(a):    Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    The employer did not use an OSHA 300A Form or equivalent:    2010 OSHA 300A Form - The employer did not record two injuries, days away from work, hours worked, and average number of employees on Summary of Work-Related Injuries and Illnesses.
