29 CFR 1928.57(b)(2)(ii): All revolving shafts, including projections such as bolts, keys, or set screws, were not guarded, except smooth shaft ends protruding less than one-half the outside diameter of the shaft and its locking means: On or about February 27, 2013, the rotating shaft of the drive line on a W.H.O. Manufacturing Tub Grinder, Model P13-60WF, serial #1101, was not guarded, exposing employees to caught-in hazards.
