29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     a) Soiled Utility Room, on or about February  27, 2013 - There were no safety glasses, or other means of eye protection provided to employees to use to prevent splashes from entering the eyes while diluting Accelerate II, a corrosive liquid.

29 CFR 1910.151(c): Where the eyes or body of any person were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Soiled Utility Room, on or about February  27, 2013 - The soiled utility room was not equipped with an eyewash station when employees diluted Accelerate II, a corrosive liquid, which contained 2-butoxyethanol, 2-Hydroxyethyl Amine, and Benzyl Alcohol .

29 CFR 1910.1030(g)(2)(iv): Annual training for all employees shall be provided within one year of their previous training:    a)  Facility, on or about April 9, 2013 - The  Bloodborne Pathogen training topic offered under Silver Chair Trainiing is offered at various times of the year, and has not been provided to occupationally exposed employees by an annual time period within thier previous training.

29 CFR 1910.1030(g)(2)(vii)(K): The employers training program did not contain an explanation of the procedure to follow if an exposure incident occurs, including the method of reporting the incident and the medical follow-up that will be made available:     a)  Facility, on or about April 9, 2013 - Post Exposure Follow-up procedures are not addressed in the Silver Chair Bloodborne Pathogen training, to include an explanation such as but not limited to the following: source patient consent, and testing and documentation that would be based on resident mental status, thus requiring express consent from either the resident or family members.
