29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): The employer is failing to protect employees from potential hazardous working conditions associated with using on site equipment without having verified through inspection or an inquiry of inspection records that it was in proper working condition. The most recent example of this was found on a residential construction located at 1405 Harney St., Omaha, NE 68102 where the employer has employees conducting interior demolition work in an industrial building and the employer allowed the employees to use an elevator that has not been inspected in over five years and as the result of this failure a faulty safety switch was neglected and resulted in an employee falling down an open elevator shaft.
