The most recent example of this was found on a residential construction located at 1405 Harney St., Omaha, NE 68102 where the employer has employees conducting interior demolition work in an industrial building and the employer allowed the employees to use an elevator that has not been inspected in over five years and as the result of this failure a faulty safety switch was neglected and resulted in an employee falling down an open elevator shaft.
