29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: On or about 2/28/13 Location: a) Spray Booth b) Main warehouse area a) The extension cord used to power the DeWalt orbital sander was damaged. The outer insulation was torn open and kinked, exposing the insulated inner conductors. b) The power cord on the Enco table saw was damaged. The cord was spliced and wrapped with electrical tape.
