29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Aleman Siding at 5015 A Minuteman Drive, Cheyenne, WY 80005: On or about February 28, 2013, the employer did not ensure that employees wore safety glasses when using pneumatic nail guns. Condition exposed employees to the hazard of projectiles to the eyes. Abatement note: Qualified safety glasses need to say Z87.1.

29 CFR 1926.451(b)(3): The front edge of the platform was not 14 inches or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling: a) Aleman Siding at 5015 A Minuteman Drive, Cheyenne, WY 80005: On or about February 28, 2013, the employer set up a pump jack scaffold with the front edge of the scaffold platform 21 inches from the face of the building that was being sided, and no guardrails were in place on the front edge. The employees were not wearing personal fall arrest systems. Conditions exposed employees to fall hazards of 14 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) Aleman Siding at 5015 A Minuteman Drive, Cheyenne, WY 80005: On or about February 28, 2013, the employer provided a pump jack scaffold for employee use that included the following deficiencies: 1) A ladder was used as horizontal guardrails, and the top edge of the top rail of the ladder was only 33 inches above the working platform. The top edge height of the top rail was less than 38 inches high as specified in 1926.451(g)(4)(ii); 2) The uprights connecting the ladder (which was used as horizontal guardrails) were not secured to the platform, and the top rail was not capable of supporting a 200 pound load as specified in 1926.451(g)(4)(vii); 3) The lower rail of the ladder, which served as the horizontal midrail, was not secured, and was not capable of supporting 150 pounds as specified in 1926.451(g)(4)(ix); 4) The scaffold had no endrails, as specified in 1926.451(g)(4)(i); The employees were not wearing personal fall arrest systems. Conditions exposed employees to fall hazards of 14 feet.
