29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) D & S Creative Communications, Inc. - Mansfield, Ohio: On or about February 13, 2013, the employer did not develop and implement a lockout program/procedures and training to protect employees from amputation injuries when servicing equipment. Employees were exposed during blade changes on the Challenge/Daeho paper cutter (s/n 52266) at a time when the machines hazardous energy was not controlled.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) D & S Creative Communications, Inc. - Mansfield, Ohio: On or about March 1, 2013, employees were exposed to injuries associated with rotating parts in that the cylinders on the backside of the Mitsubishi Offset Press (No. 1F221) coater unit were not guarded. The employer did not assure that the attached guard was operated and positioned to protect press operators.
