29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: On February 26, 2013, an employee was exposed to the fall of hazards from an elevation of appoximately 15 feet to the ground below, while applying stucco from the frames of the scaffold.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The endpoints at each upright were more than 48 inches (1.3 m) apart: On February 26, 2013, an employee was exposed to fall hazards from an elevation of approximately 15 feet in height from the ground level while applying stucco from a scaffolding system without either a toprail or midrail.
