Adequate strain relief was not incorporated into the fan / switch box wiring system. B.) Keystone Foundry Division of United Brass Works, 944 West 12th Street, Erie, Pennsylvania, on or about March 1, 2013 and times prior thereto: A metal wall switchbox was wired to the flexible cord of a Dayton 30 Commercial Duty fan. The switchbox was designed with metal knockouts and was not approved and/or listed and labeled for the fan application.
