29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. On or about March 1, 2013, at the above addressed jobsite, an employee was permitted to work near unprotected electric utility lines, thereby exposing the employee to the hazards associated with electric shock. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about March 1, 2013 at the above addressed jobsite, employees engaged in roofing operations on a residential building were not protected from falls of approximately 11 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
