29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Southeast section of the grocery store, by the cheese display cooler, two flexible cords were plugged into a three-way electrical receptacle device, which was plugged into a 110 volt receptacle. The cords and three-way receptacle were only two wire devices, they did not have a separate electrical wire for the grounding path.
