29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Entire Facility. Maintenance mechanic(s) performed servicing and maintenance on machines or equipment with multiple energy sources such as but not limited to CNC milling and drilling machines. The employer did not implement and maintain a lockout/tagout program consisting of specific energy control procedures, employee training and periodic inspections as follows: a) Procedures for the control of hazardous energy were not developed, documented and utilized by the employer. b) Training of "Authorized", "Affected" and "Other" employees was not conducted an certified by the employer. Employees authorized to perform lockout/tagout were not informed of and trained to control hazardous energy sources according to specific procedures. c) Periodic inspections of the energy control procedures were not conducted and certified by the employer at least annually to ensure that the procedures and requirements of the standard were being followed. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The employer did not ensure that employees who were authorized to operate the powered industrial truck received training and a performance evaluation (driving test) before operating the vehicle in the workplace. No abatement documentation or certification is required for this item. Abatement note: After initial training, each authorized operator must receive a performance evaluation (driving test) every three years. The employer must certify (document) that training and performance evaluations have been completed.
