29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:    a) At Craftmaster Printers:  On or about February 25, 2013 and at times prior ; the employer exposed his employees to smoke inhalation and fire hazards in that the employer required employees to attempt to extinguish incipient stage fires and had not trained employees on how to use fire extinguishers and on how to fight incipent stage fires.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a)  At Craftmaster Printers:  On or about February 25, 2013 and at times prior; the employer exposed his employees to struck-by hazards in that an employee was allowed to operate a powered industrial truck without the employer first ensuring the employee was competent to operate the equipment as demonstrated by the completion of the required training and evaluation specified in the regulation.

29 CFR 1910.178(l)(4)(iii):  An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    a)  At Craftmaster Printers:  On or about February 25, 2013 and at times prior; the employer allowed his employees to operate a powered industrial truck without the employer insuring that the operators had received the required evaluation at least every three years as specified in the regulation.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a)  At Craftmaster Printers:  On or about February 25, 2013 and at times prior; the employer exposed his employees to hazardous chemicals in that employees were allowed to use hazardous chemicals in their work area without first being provided information and training on inks and other hazardous chemicals.
