29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about February 28, 2013, an employee was exposed to struck by hazards, when another employee was working above at approximately 15 feet from the ground level, while using a pneumatic nail gun for nailing the asphalt shingles.

29 CFR 1926.501 (b)(13) Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about February 28, 2013, employee was exposed to falls of approximately 15 feet in height to the ground level below, when working with no fall protection, while installing asphalt shingles to a side roof area of the balcony.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: On or about February 28, 2013, employees were exposed to falls of approximately 15 feet to the ground level below, while using two extension ladders, where one extended approximately 6 inches and the other 1 foot beyond the landing, to access the roof of the residential home.
