29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility, 300 C South 1st Street, Connellsville, PA: On or about 03/04/13 the employer did not establish an energy control program consisting of procedures, employee training and periodic inspections to ensure that before an employee performed servicing or maintenance the equipment or machine was isolated from the energy source. Employees maintain equipment such as the Eagle CNC and Amada band saw and the employer does not have an energy control program to provide a procedure to follow to isolate energy sources nor was any employee training provided.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from not inspecting the hoists to ensure they were in safe operation: (a) Facility, 300 C South 1st Street, Connellsville, PA: On or about 03/04/13 the employer did not perform frequent or periodic inspections of the Budgit 1/2 ton hoist to ensure the mechanisms, upper limit device, brakes, latches, etc. were in proper working order. Employees use the hoist daily to move parts and products into the blasting room. (b) Facility, 300 C South 1st Street, Connellsville, PA: On or about 03/04/13 the employer did not perform frequent or periodic inspections of the Chicago hoist, lifting capacity 500 lbs., to ensure the mechanisms, upper limit device, brakes, latches, etc. were in proper working order. The latch on the hook was twisted and did not function. The employees use the hoist to move parts onto the plasma cutter.
