29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) Jobsite: The working levels of the tubular welded scaffold were not fully decked between the front and rear uprights. Employees applying caulk from the 2nd and 3rd levels were exposed to fall hazards ranging from twelve (12) to twenty (20) feet. On or about March 1, 2013.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. a) Jobsite: A ladder or other safe means of access was not provided to employees working on the tubular welded scaffold. Employees were climbing the side frames of the scaffold to access/egress the 2nd and 3rd working levels. On or about March 1, 2013
