29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) Worksite:  On or about February 1, 2013, an employee was not wearing a body belt with lanyard while performing demolition operations from a JLG Lift, exposing the employee to an approximate 20' fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a) Worksite: On or about February 1, 2013, Employees were not trained by a qualified person to recognize the hazards associated with aerial lifts and to understand the procedures to control or minimize those hazards. An employee was working from a JLG aerial lift approximately 20' above ground level without fall protection.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.  (a) Worksite, Ridge Ave:  On or about February 1, 2013, an engineering survey was not performed by a competent person proir to permitting employees to start demolition operations of a former car dealership. The employer did not have in writing, evidence that a survey was performed.
