29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) On or about 3/1/13 in the Fitting department; before blade changing on bandsaw WC2010 the two access doors ( which have safety switches ) are opened without the use of a lockout or tagout device on the energy isolation device.        b) On or about 3/1/13 in the Fitting department; before changing the jaws on the Colatch machines, such as but not limited to #78 and #3, the power switch is turned off and a hasp applied and the air supply is switched off without the use of a lockout or tagout device.       c) On or about 3/1/13 in the Elbow department; before change over and set up of the tooling on the elbow machines, the emergency stop button is depressed and the machine controls are placed in manual without the use of a lockout or tagout device.
