29 CFR 1910.29(a)(4)(iii): Where leveling of the elevated work platform is required, screw jacks or other suitable means for adjusting the height shall be provided in the base section of each mobile unit. Location: At the Keystone Automotive Industries facility at 275 Corporate Woods Place, Bridgeton, MO 63044. A suitable means for leveling the yellow mobile ladder stand was not provided due to the damaged screw on the left front of the ladder stand. The ladder stand was approximately 90" in height from the ground to the platform and was unstable due to the damaged screw which prevented users from being able to level the equipment. Due to the damaged mobile ladder, the workers were climbing the racking system in order to reach the upper shelves to access material.

The metal conduit enclosure for electric wires on circuit # 28.30.32, was pulled away from the junction box exposing the wires. The service provided power to the battery charger for powered industrial vehicles.
