29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder: (a) Levy Environmental Services Company DBA Fulton Mill Services on or about February 25, 2013, at the address of 6565 County Rd. 9 Delta, OH: The employer failed to assure employees were provided with and /or utilized an acceptable means of fall protection while switching out screening plates atop screen deck #2 of the slag processing plant. Employees performing this task were exposed to a 25 foot fall to the ground below.
