29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. Maintenance Shop and Prep Area: On or about February 21, 2013, the employer did not ensure that fire extinguishers were mounted where there was a fire extinguisher sign identifying where one should be.

29 CFR 1910.215(a)(4): Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. Prep Dept.: The clearance between the abrasive wheel and work rest was 1" on a 6" Black and Decker grinder (serial # unclear). Maintenance Shop: On a 6" Dewalt grinder (serial #200348), the distance between the abrasive wheel and work rest was 1". Employees are exposed to the hazard of being struck-by flying objects.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral memeber at the top exceeded one fourth inch: Prep Area: On a Dewalt 6" grinder (serial #200348). The clearance between the abrasive wheel and metal housing was 1 3/4". Maintenance Shop: On a 6" Black and Decker grinder the distance between the abrasive wheel and metal housing was 1". Employees are exposed to hazard of being struck by flying fragments in the event the abrasive wheel is shattered during grinding operations.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Main Fab Shop (east wall): On or about February 21, 2013, the employer did not ensure that flexible cord sets (extension cords) were visually inspected. The outer jacket of a power cord connected to a Dewalt grinder was damaged and exposed the metal wiring. Employees were exposed to an electric shock hazard.
