29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location: (a) Powder Coating Area-Employees are required to clean the inside of the powder coating booth with a Nilfisk 118 vacuum cleaner. On 4/16/13, dust was collected inside the booth while the employees were cleaning. The dust collected tested as a class II dust (explosive). The electrical vacuum cleaner used was not approved for the location. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards: a)Metcam, Inc.- The employer did not ensure that the ventilation exhaust fans (verti-blowers) at the new paint line oven were covered or isolated to prevent employees from contacting parts with surface temperatures of 185 degrees F to 245 degrees F while working and walking adjacent to the oven area on or about 2/12/13. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

The nearest eye wash for employees adding this chemical to the washer was not located for immediate emergency use in that it was behind a wall on or about 2/12/13.
