29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       a.  On or about 2/27/13, F82 Machine Line, There was no guard to prevent employees from contacting a pinch point at the feeders backstop which exposes employee to caught-by hazards.     b.  On or about 2/27/13, F110 Machine Line, rotary die area, there was no machine guarding to prevent contact with rotating parts such as the floor level waste conveyor, and die cylinder/roller which exposes employees to caught-by hazards.     c.   On or about 2/27/13, Apstar 1628 Machine Line, AG Stacker layboy area, there was no machine guarding to prevent contact with rotating parts such the floor level waste conveyor, belts, and pulleys, and snubbing wheels which exposes employees to caught-by hazards.     d.  On or about 2/27/13, R110 Machine Line, AG Stacker layboy area, there was no machine guarding to prevent contact with rotating parts such as the floor level waste conveyor, belts, and pulleys which exposes employees to caught-by hazards.     e.  On or about 2/27/13, P4115 Machine Line, folding conveyor area lacked adequate machine guarding to prevent contact with rotating parts such as belts and rollers which exposes employees to caught-by hazards.     f.  On or about 2/28/13, P140 (Curioni Lodi) Machine Line, lacks guarding to prevent contact with rotating slotter heads (knives), and belts and rollers at the front of the stacker conveyor table.     g.  On or about 2/28/13, P140 (Curioni Lodi) Machine Line, Greene Line Stacker end table lacks guarding to prevent downward movement of the inclined conveyor when someone is underneath it and exposes employees to struck-by hazards.     h.  On or about 2/28/13, P78 (Langston) Machine Line, lacks guarding to prevent contact with rotating slotter heads (knives), and belts and rollers at the front of the stacker conveyor table     i.  On or about 2/28/13, P78 Machine Line, Greene Line Stacker inclined conveyor lacks guarding to prevent downward movement of the inclined conveyor when someone is underneath it and exposes employees to struck-by hazards.     j.  On or about 3/7/13, Bostitch Automatic Stitcher Machine lacked guarding for a guide roller shaft which was about 56 inches above the floor and exposes employees to caught-by hazards.      k.  On or about 3/7/13, Bostitch Bliss Wire Stitcher, lacked guarding for the foot pedal of the wire stitcher which exposes employees to struck-by hazards.
