29 CFR 1910.25(d)(2)(xii): Tops of the ordinary types of stepladders shall not be used as steps;  a.  At the workplace, in the warehouse area, on or about March 5, 2013, an employee was standing on the top cap of a 6 foot wooden A-frame self-supporting ladder accessing product from the top shelf of a storage rack exposing the employee to an approximately 6 foot fall to the concrete floor below.

29 CFR 1910.37(a)(3):   Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.   a.  At the workplace, in the warehouse, on the South wall,  the employer did not maintain the emergency exit free of obstruction.  The exit route was blocked by pallets and boxes of products.

At the workplace, in the warehouse, at the shipping and receiving dock, a Power Factor, model # 1PF18A-520SS, Serial # 111343, battery charging apparatus was not protected from damage by the forklift.
