29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: On or about February 25, 2013 in Department 140, Barrel Cell: An employee sustained contusions and lacerations to the left hand, caused by contact with the moving push rod assembly on the Modern Machine Tool Company automatic bar feeder. The movable guard had been opened during operation. The guard was not affixed to prevent access to the danger zone by fasteners or an interlock.
