29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: (a) Location - Rt 113, Groveland, MA: On or about 4/6/13, tree trimming was performed within 10 feet from engergized overhead power lines, without contacting the power company to de-energize the lines, or without taking any other protective measures to prevent electric shock type injuries.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: (a) Location - Rt 113, Groveland, MA: On or about 3/6/13, the employer failed to ensure an employee was protected from falls while working from a bucket truck aerial lift. Northern Tree Service Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910. 67 (c) (v) which was contained in OSHA Inspection Number 313800732, Citation Number 01, Item Number 1 Issued 10/29/10 with respect to workplace located at East Broadway/William J Day Boulevard Boston, MA .
