29 CFR 1910.1030(h)(5)(i)(A): The employer did not maintain a sharps injury log that contained at a minimum the type and brand of device involved in the incident:  a) Two sharps exposure incidents occurred in 2011 which involved Personal Care Assistants.  The OSHA 300 Log of Work - Related Injuries and Illnesses, the Employee Incident Reports, nor any of the supporting documents referenced in the facility's Exposure Control Plan did not specify the type and brand of device involved in the incidents, on or about March 5, 2013.   ABATEMENT CERTIFICATION REQUIRED
