29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) At the site located at 712 West Drew St, Houston, TX: On March 4, 2013 an employee was assigned to work at a height of 13 feet 6 inches above a lower level on decking that was not fully planked, exposing the employee to a fall hazard. Ramirez Stucco was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 314416975, Citation Number 01, Item number 001 and was affirmed as a final order on June 29, 2010, with respect to a workplace located at 1402 Spring Street, Houston, TX.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. At the site located at 712 West Drew St, Houston, TX: On March 4, 2013, an employee assigned to work on scaffold platforms up to 4 stories high was not provided a ladder or similar access for transit up to and between levels. The employee was observed climbing the cross braces between 13 and 20 feet above the ground, exposing the employee to a fall hazard. Ramirez Stucco was previously cited for a repeat violation of this occupational safety and health standard which was contained in OSHA inspection number 314416975, Citation Number 01, Item Number 001 and was affirmed as a final order on June 29, 2010, with respect to a workplace located at 1402 Spring Street, Houston, TX.
