29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Christian Living Communities, at 2479 S Clermont Street, Denver, CO:  On and preceding 3/6/13, Christian Living Communities did not ensure that procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  One employee operated the Marathon Equipment Company Ramjet trash compactor.  The compactor had two energy sources, electrical and hydraulic.  The employee entered the trash compactor to unclog trash from the hydraulic ram.  The hydraulic and electrical power sources to the trash compactor were not de-energized or controlled during the entry.  The employer had not developed, documented, or utilized procedures for the control of the hazardous energy sources.  This condition exposed the employee to an uncontrolled energy hazard.    (b) Christian Living Communities, at 2479 S Clermont Street, Denver, CO:  On and preceding 3/6/13, Christian Living Communities did not ensure that procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  One employee performed maintenance on the Trane heat pump #55.  The heat pump had two energy sources, electrical and pressurized hot water.  The employer had not developed, documented, or utilized procedures for the control of the hazardous energy sources.  This condition exposed the employee to an uncontrolled energy hazard.    (c) Christian Living Communities, at 2479 S Clermont Street, Denver, CO:  On and preceding 3/6/13, Christian Living Communities did not ensure that procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  Contractors service the Raypak domestic hot water heaters and Ajax model WG-1375 building heat boilers.  The heaters and boiler had multiple energy sources, electrical and compressed gas.  The employer had not developed, documented, or utilized procedures for the control of the hazardous energy sources.  This condition exposed employees and contractors to an uncontrolled energy hazard.

Certified Nursing Assistants (CNA's) used disposable razors while caring for patients and residents of a skilled nursing center.  After use, the contaminated sharps were carried from the interior of the resident's room, through the door and to the med cart, up to ten feet away, where the contaminated sharp was disposed in a sharps container after unlocking the cart cabinet door, or taken to the nurse's station, where the contaminated sharp was given to the nurse for disposal into the sharps container in the locked med room.  The employer did not ensure that sharps containers were located as close as feasible to where the sharps were used.  This condition exposed employees to a bloodborne pathogen hazard. The employer had not certified that a personal protective equipment hazard assessment had been performed.  Employees were exposed to the following, but not limited to, hazards:  1.  Exposure to Strategic Resource Optimization Inc Electro Biocide (containing 0.02% chlorine dioxide) when deep cleaning resident rooms; 2. One employee whom was required to wear a respirator had facial hair, including a goatee.
