29 CFR 1904.39(a): The employer did not orally report by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA) the death of an employee within 8 hours after the death or the in-patient hospitalization of three or more employees as the result of a work-related incident: On or about March 4, 2013, an employee sustained multiple blunt force trauma injuries from a fall resulting in the employee's death. The employer did not notify OSHA of the employee death within the required 8 hours.
