29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited,  did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall of approximately fourteen feet to the ground below while installing vapor barrier.    b) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited,  did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall of approximately ten feet to the ground below while working in and around unguarded window openings.    c) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited,  did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall of approximately ten feet to the ground below while working in and around unguarded floor openings.

29 CFR 1926.503(a)(1):     The employer did not provide an effective training program for each employee who was exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:        a) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited, did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall of approximately fourteen feet to the ground below while installing vapor barrier due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.    b) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited, did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall of approximately ten feet to the ground below while working in and around unguarded window openings due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.     c) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited, did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall of approximately ten feet to the ground below while working in and around unguarded floor opening due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.

29 CFR 1926.1052(c)(1)(i):     Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:      a) Construction Site: On or about March 06, 2013, and at times prior thereto, as the controlling employer, Cadius Partners Limited, did not ensure its sub-contractor, Jupe Construction LLC, protected their employees in that employees were exposed to a fall to the floor below using a stairway with approximately seventeen risers to access/egress the second story from/to the ground level to retrieve tools and material.
