29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    At this workplace, employees conducted overhand bricklaying operations on a two story residential exterior wall from a scaffold at a height greater than eleven feet above ground level and the uprights did not bear on baseplates.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   At this workplace, employees conducted overhand brick laying operations on a two story residential exterior wall and climbed the scaffold to access the working platform; a ladder for safe access was not provided.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).   At this workplace, employees conducted overhand bricklaying operations on a two story residential exterior wall from scaffolding at a height greater than eleven feet above ground level and a fall protection system or guardrail system was not in place.
