29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid point: a) 600 Sylvan Ave., Englewood Cliffs NJ: An employee was exposed to a fall of approximately 10 feet while using a ladder that did not extend 3 feet above the landing paltform, on or about 2/25/2013.
