29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. (a) On or about 3/5/13 at 17 Mercer Street, Cincinnati, Ohio, the employer failed to ensure that holes in decking were covered to protected employees from tripping or stepping into opening. Employees were exposed to a 29 inch drop to a lower platform that was unguarded and opened to a 30 foot drop.
