29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: Location: Front of building exit stairs. On or about 2/28/13; a) Employees were exposed to a fall while accessing stairs that were missing a handrail.

29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. Location: 2nd floor rear exit door. On or about 2/28/13; a) The rear exit door was maintained locked from the inside with two padlocks.
