29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: a) On or about 02/28/13, at 31 Gonzalez Clemente Avenue, Mayaguez, PR, in the Mission Support office area, four relocatable power taps were not directly connected to a permanently installed branch circuit receptacle. b) On or about 02/28/13, at 31 Gonzalez Clemente Avenue, Mayaguez, PR, in the Director's office area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle. c) On or about 02/28/13, at 31 Gonzalez Clemente Avenue, Mayaguez, PR, in the kitchen area, a ground-fault circuit interrupter was not installed in the 125-volt, single phase, 15- or 20-ampere receptacle installed within 6 feet or 1.8 meters of the outside edge of a sink. d) On or about 02/28/13, at 31 Gonzalez Clemente Avenue, Mayaguez, PR, in the Conference Room area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle. e) On or about 02/28/13, at 31 Gonzalez Clemente Avenue, Mayaguez, PR, in the LAN room area, three relocatable power taps were not directly connected to a permanently installed branch circuit receptacle. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
