29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):  (a) Snows Concrete Forming at 3528 & 3530 Tennyson St., Denver, CO 80212:  On and before March 5, 2014, the employer did not ensure that the sloping of the east end of the excavation was in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4).  Employees entered the excavation and worked adjacent to the east face that was improperly sloped for Type C soil.  This condition exposed employees to cave-in and struck-by hazards.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    (a) Snows Concrete Forming at 3528 & 3530 Tennyson St., Denver, CO 80212:  On and before March 5, 2014, the employer did not ensure that employees used a ladder in a manner for which it was designed in that an approximate 10 foot tall step ladder was used as an extension ladder as a means of access to the top of concrete forms being set for a basement foundation.  This condition exposed the employees to a fall hazard of 10-12 feet.
