Employees entered the excavation and worked adjacent to the east face that was improperly sloped for Type C soil.  This condition exposed employees to cave-in and struck-by hazards.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    (a) Snows Concrete Forming at 3528 & 3530 Tennyson St., Denver, CO 80212:  On and before March 5, 2014, the employer did not ensure that employees used a ladder in a manner for which it was designed in that an approximate 10 foot tall step ladder was used as an extension ladder as a means of access to the top of concrete forms being set for a basement foundation.  This condition exposed the employees to a fall hazard of 10-12 feet.
