29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:     This violation was observed on February  26, 2013, at an establishment located at 920 West White Street, Anna, Texas: on the production areas where employees were exposed to the hazards of fire, deflagration, and explosion from the accumulation of a class II combustible saw dust had accumulated on equipment, walls and ledges at depths of up to (3)inches.

29 CFR 1910.307(c)(2)(i):  Equipment was not approved for the class of location, and for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was present:     This violation occurred on January 26, 2013, at a work place located at 920 West White St. Suite 109 Anna, Texas 75409 ; on the production area where the employer did not ensure that equipment is approved for the class of location, and for the ignitable or combustible properties of the saw dust:     a) Employees operated a Dewalt Chop Saw and other power tools in the Custom Wood Work Area which dispensed saw dust (Class II combustible dust). The operation of the Dewalt Chop Saws and other power tools created the release of the saw dust intermittently throughout the day. General purpose wiring, breaker panel boxes, disconnect switches, and receptacles were not dust-tight ignition proof and approved for Class II  locations.     b)Employees operated a Dewalt Chop Saw and other power tools in the Line Bore Area which dispensed saw dust (Class II combustible dust). The operation of the Dewalt Chop Saws and other power tools created the release of the saw dust intermittently throughout the day. General purpose wiring, breaker panel boxes, disconnect switches, and receptacles were not dust-tight ignition proof and approved for Class II  locations.     c) Employees operated an Onsurd Router and other automatic tools in Bay 4 which dispensed saw dust (Class II combustible dust). The operation of the Dewalt Chop Saws and other power tools created the release of the saw dust intermittently throughout the day. General purpose wiring, breaker panel boxes, disconnect switches, and receptacles were not dust-tight ignition proof and approved for Class II  locations.
