29 CFR 1910.215(a)(2): Guard design. The safety guard shall cover the spindle end, nut, and flange projections. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard, except: 1. Inside of the wood shop, on the east wall, an employee was using a Dewalt grinder with no guards in place on the side of each grinding wheel. The right wheel was made out of fiber, and the left wheel was made of the regular stone. Employees were exposed to struck by and amputation injuries.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles may have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 2.44 m (8.0 ft) above the floor may have exposed terminals. 1. Near the receiving dock, on the west wall left of the exit door, there was a cracked (broken) bottom receptacle on the duplex outlet.
