29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: (a) Jobsite, 412 Hazel Avenue, Butler, PA: On or about 03/07/13 a safe means of access was not provided to the platform that was approximately 21 feet above the ground. The employees did not have an independent ladder to use to access the platform and climbed one of the ladders used to support the platform to get to the working level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) Jobsite, 412 Hazel Avenue, Butler, PA: On or about 03/07/13 two employees were working from an 8 in 12 pitched roof of a residential structure removing slate shingles without utilizing any fall protection systems. The fall hazard to the employees ranged from approximately 23-25 feet to the ground below.
