29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    At the site where employees were using an extension cord to provide power to their saws, the insulation was pulled back exposing employees to electrical hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     At the site employees were framing a two story, single-family residence without the use of fall protection at an approximate height of 22 feet, exposing employees to fall hazards.
