(a) In the material storage yard, the employer did not ensure that aisles and passageways were free from snow and/or ice accumulations. Powered industrial vehicles, such as but not limited to the Toyota, Mitsubishi, or Hyster, were getting stuck or sliding on snow and/or ice.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) The employer did not perform a certified hazard assessment to determine personal protective equipment requirements. Employees were performing liquid propane gas tank changes on the forklifts, such as but not limited to on the Toyota, Mitsubishi, or Hyster without the use of adequate eye protection.
