29 CFR 1910.178(p)(1):     Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):    (a) Furnace Room:  The TCM propane powered forklift, the short mast forklift, did not have a nonslip surface on the brake pedal and was not taken out of service.    (b) Blade Room:  The TCM FCG25 propane powered forklift did not have a nonslip surface on the brake pedal and was not taken out of service.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    (a) Blade Room:  The Jet Bench grinder tongue guards were adjusted at greater than one fourth inch on both the left and right grinding wheels.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a)  The Vertical Load/Unload Table in the Temper Room had a rectangular electrical box of more than 50 volts that was not guarded with a cover.    (b)  The Vertical Load/Unload Table by the LIPE Press had an oval electrical box of more than 50 volts that was not guarded with a cover.

29 CFR 1910.305(a)(1)(i):     Metal raceways, cable armor, and other metal enclosures for conductors were not connected to all boxes, fittings, and cabinets to provide effective electrical continuity:    (a) The Vertical Load/Unload Table in the Temper Room did not have effective electrical continuity as the electrical metal conduit was not connected from the control to the junction box on the table.     (b) The Vertical Load/Unload Table in the LIPE Press area did not have effective electrical continuity as the electrical metal conduit was not connected from the control to the oval junction box, and from the oval junction box to the metal conduit beneath it.
