29 CFR 1926.850(a):   An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations:  At this site, the employer did not perform planning of the partial demolition of an existing concrete box culvert (stormwater drain).  Collapse hazards were not evaluated and addressed for employees performing saw cutting and removal of the box while standing on its structure.  The employer did not develop a survey or demolition plan with engineering input nor develop job safety analysis (JSA) prior to work.

29 CFR 1926.501(b)(15): Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  At this site, the employer did not protect employees performing concrete sawing into the work deck on which they were standing, from a fall of seven feet or greater to the floor of the concrete box culvert (storm water drain).  Employees were neither removed from the hazard zone nor provided a fall protection system.
