29 CFR 1926.20(b)(2): The employer did not initiate and maintain a program of frequent and regular inspections made by competent persons at the jobsite:       a)  During concrete pumping operations the employer did not inspect (measure) the pipe wall thickness with an ultrasonic gage or caliper gage to ensure that the pipe walls are not worn thin from abrasion by the concrete passing through the pipe.  Employees were exposed to pipe burst hazards and being struck by wet concrete sprayed from the pipe.        Location:  WTC construction site, building 4, New York, N.Y.      On or about 11-28-2012      NOTE: BECAUSE THIS ITEMM WAS CORRECTED DURING THE INSPECTION THE EMPLOYER NEED NOT SUBMIT ABATEMENT CERTIFICATION NOR DOCUMENTATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
